FHYFC REGISTRATION

To register for Forest Hills Youth Football & Cheerleading, please complete the following steps;

1.) Please review the Forest Hills Youth Football & Cheerleading Fact Sheets. "FOOTBALL" - "CHEERLEADING"

2.) Print and complete either the football or cheerleading registration form. "FOOTBALL" - "CHEERLEADING"

3.) Print and complete the Media Waiver form. "MEDIA WAIVER"

4.) Print and complete the Medical Release form. "MEDICAL RELEASE"

5.) Registration Fees are as follows; 1st child $50.00, 2nd child $35.00, 3rd and subsequent children $20.00

6.) Enclose the above forms along with a check payable to FHYF in an envelope and mail to:

Forest Hills Youth Football & Cheerleading held open registration on Sunday January 20th. While registration remains open to all, each team has the right to close registration at any time due to limited available roster openings.
